TYPE 2 DIABETES (T2DM)

MANAGEMENT

All patients with Type 2 Diabetes (T2DM) are eligible for a GP referral to participate in allied health exercise-based group programs and education through the Referral for Group Allied Health Services under Medicare for patients with Type 2 Diabetes.

An individualised exercise program for Diabetes helps to:

  • Improve muscle response to insulin

  • Improve fasting blood glucose

  • Increase glucose uptake through effective muscle movement

  • Reduce the need for medication

  • Manage and reduce other co-morbidities

  • Maintain a healthy weight

  • Enhance patient education

  • Provide a supportive and social environment for treatment

  • If you have been diagnosed with Type 2 Diabetes, you are eligible for our Diabetes Group Exercise Program.

    You will require a referral from your GP for ‘Group Allied Health Services under Medicare for patients with Type 2 Diabetes”

    Have your GP clinic send through your referral or simply call us to book your appointment and bring your referral on the day.

    • 1x initial 1:1 assessment with an Accredited Exercise Physiologist

    • 8x 1hr group exercise sessions including:

      • Behavioural change guidance

      • Tailored exercise prescription

      • Self-management education

      • Home exercise guidance

    • Initial and final reports sent to referring GP
