TYPE 2 DIABETES (T2DM)
MANAGEMENT
All patients with Type 2 Diabetes (T2DM) are eligible for a GP referral to participate in allied health exercise-based group programs and education through the Referral for Group Allied Health Services under Medicare for patients with Type 2 Diabetes.
An individualised exercise program for Diabetes helps to:
Improve muscle response to insulin
Improve fasting blood glucose
Increase glucose uptake through effective muscle movement
Reduce the need for medication
Manage and reduce other co-morbidities
Maintain a healthy weight
Enhance patient education
Provide a supportive and social environment for treatment

If you have been diagnosed with Type 2 Diabetes, you are eligible for our Diabetes Group Exercise Program.
You will require a referral from your GP for ‘Group Allied Health Services under Medicare for patients with Type 2 Diabetes”

1x initial 1:1 assessment with an Accredited Exercise Physiologist
8x 1hr group exercise sessions including:
Behavioural change guidance
Tailored exercise prescription
Self-management education
Home exercise guidance
Initial and final reports sent to referring GP
